What type of breast implants does Dr. Bukret use?

What kind of incisions do we use?
Dr. Bukret uses periareolar inferior (lower half of the areola) or submammary approach for most of the cases; although, for some cases, we prefer an armpit incision according to the anatomy of the patient and previous surgeries.
Why we don’t use textured breast implants anymore?
We opted for the use of smooth gel silicone breast implants after the controversy generated by the appearance of cases of anaplastic large cell lymphoma (ALCL) in the breast that was related to textured implants. Although relevant statistical studies are not sufficient, the available clinical data that exist warn about this possibility making it more advisable for now to use this type of breast implants.
However, currently, the recommendation for breast augmentation is to use smooth breast implants and change them every 7 to 10 years.
The incidence of ALCL is in the order of 1.1 per hundred thousand (FDA MAUDE data and Plastic Surgery Foundation ASPS / FDA Profile database). There is an average time of 8.6 years between implant placement and diagnosis of the disease, and the mean age of presentation is 54 years. 78.3% present as a late seroma, 36.7% as a tumor, and 14.9% with compromised lymph nodes. Regarding the characteristics of the implants, they have been either rough in 51.7% of the cases or of unknown texture in the remaining 48.3%. There are about 200 histologically confirmed cases in the world.

Previously, textured silicone gel implants were preferred because a lower incidence of capsular contracture supposed to be lower in comparison to smooth implants. Capsular contracture is one of the main reasons to replace breast implants in the long term; therefore, textured implants prevent long-term capsular contracture and significantly decrease the need for breast implant replacement.
Which advantages do periareolar incision has compared to submammary or armpit incisions?
The main advantages of the periareolar incision are due to the excellent visibility during surgery; therefore this permits better control of symmetry avoiding the more common complications; the quality of the scar is excellent in most cases, and does not interfere with breastfeeding after surgery.
Which plane does Dr. Bukret use to place the breast implants?

Should I replace the implants after a time?
Breast implant replacement is necessary at least every 10 years or when there is a complication like capsular contracture, which can cause you symptoms like chronic pain. Other reasons for replacing implants are shape distortions, asymmetries, or infrequently rupture of implants.
How long does breast augmentation surgery take?
It takes 1 to 3 hours.
What kind of anesthesia is used for breast augmentation?
Dr. Bukret generally uses deep sedation (technically known as neuroleptoanesthesia) for most breast augmentation. This is always used in conjunction with a local anesthetic to reduce discomfort during and after surgery.

Breast augmentation with fat grafting is indicated for patients who desire a subtle and natural breast augmentation or “boob job” without using breast implants. The main condition is to have enough fat to transfer from other areas; an additional benefit is obtained by laser liposculpture
How long is the recovery time after breast augmentation with implants or fat transfer?
Breast augmentation by fat grafting has a shorter recovery period and less discomfort compared to breast augmentation with implants.
After having breast augmentation with implants you should expect to be doing normal activities after 10 to 14 days; the submuscular approach produces more discomfort and recovery could take longer.
Which are the post-operative care measures and treatment?
