Plastic surgery Frequently Asked Questions

Plastic Surgery in general

What is the difference between cosmetic and reconstructive surgery?

Cosmetic or aesthetic plastic surgery is performed to enhance or change a healthy, normal, functioning part of the body. Nothing but the patient´s desire for physical improvement necessitates cosmetic plastic surgery. Reconstructive surgery, however, is intended to correct a physical abnormality caused by a birth defect, disease or tumor, physical trauma, or infection. The goal of reconstructive surgery may be to restore function or to achieve physical normality. Reconstructive surgery addresses problems such as of breast cancer by reconstructing the affected breast, a cleft lip, or having surgery to repair a face damaged by fire.

What is recovery from plastic surgery like? Will I be able to tolerate the pain post-operatively?

Each patient will tolerate pain post-operatively in a different way, and we consider this. While some patients may describe the pain as an ache, others experience greater discomfort. Appropriate pain medications are prescribed for the post-operative patients, and these help minimize pain or discomfort. Most facial surgical procedures have minimal discomfort post-operatively. Liposuction is slightly more uncomfortable, and plastic surgeries that require elevation or tightening of the muscles-such as an abdominoplasty or breast augmentation have discomfort equal to that of a C-section.

How long is the recovery period and when can I return to work?

The length of time it takes to recover after plastic surgery varies depending on the procedure performed and the person operated on. Most patients will require assistance for the first two days. Then most patients are able to care for themselves, but may still need assistance if they have small children to care for.
In most cases you will be able to get back to work within a week, liposuction though does not require any rest at all yet the first 3 days after liposuction you will be quite sore.

The time a patient resumes regular exercise or straining activities varies based on the plastic surgery performed. All patients are encouraged to start a slow walking routine on the second postoperative day. Regular aerobic and more vigorous activities are not allowed during the first 2 weeks in order to decrease the risks of bleeding, swelling, and bruising. Weight lifting and contact sports are allowed at 1 month in most cases.

Who are the best candidates for plastic surgery in general?

Individuals; consenting adults who are in general good health and wish to enhance any part of their body.

Why are a lot of people against plastic surgery (cosmetic surgery)?

There are a number of reasons why a great number of people are against cosmetic surgery.

Many are simply afraid to die or surgery outcome going awry. Many are scared in general due to lack of information or still believe the plastic surgery of today hasn´t evolved since the 70´s for the same reason.

Plastic surgery has remained taboo even in 21st century. Women are still criticized (usually by other women) about undergoing cosmetic surgery. But why are they criticized? Being plastic surgery not affordable by everyone, women secretly wish they could enhance certain parts of their body yet they wouldn’t even acknowledge so to themselves or if they did, they wouldn´t bother to make the sacrifice it sometimes implies.

However there is always the common possibility of a woman´s spouse or significant other to disagree with her undergoing for example breast augmentation, due to the persistent and latent sexism of today’s society. It is not uncommon for a man to feel threatened by his spouse having fuller breasts and thus being perhaps more attractive.

As a general note, provided that plastic surgery is done for one’s own satisfaction and not to please any other people, it is important to be prepared for relative’s and friend’s negative reactions. Being this said, your decision to undergo plastic surgery has to be informed and firm enough as it isn´t really anyone´s business but the person going through it.

Rhinoplasty (Nose plastic surgery)

Is there a lot of bruising afterwards, and how long will it last?

This depends again on how complex the procedure is. Some patients have little or no bruising. A few experience five to seven days of purple discoloration and swelling around the eyes and upper cheeks. The nose itself will be swollen for around seven to fourteen days, but subtle resolution of the swelling at the tip of the nose can take months after surgery to go away.

Where are the incisions placed during nose surgery?

In general, most or all of the incisions in rhinoplasty are hidden in the inside of the nostril. While external incisions hidden in the creases where the nose meets the cheek can be used to narrow the nose, incisions on the bridge or tip are not used in cosmetic surgery of the nose.

Will my nose grow after surgery?

In general, our noses tend to elongate as we enter the middle years of our life and drooping of the tip of the nose can occur in the later years as elasticity disappears from the body. While undergoing a cosmetic rhinoplasty as a teenager or young adult will not prevent the effects of aging on the nose, the final shape that results from this surgery is expected to last a lifetime and regrowth of bumps and deformities corrected by the operation is an extremely rare occurrence.

I have trouble breathing through my nose and have sinus problems.

The condition described is usually associated with a deviated septum or a bent internal frame of the nose. Most of the time this results from trauma, such as a broken nose.

What Is a Septoplasty?

Septoplasty is non-cosmetic plastic surgery of the nose designed to help with breathing. It is sometimes done at the same time as a rhinoplasty. During a septoplasty procedure, the shape of inside of the nose (the septum, which separates the two breathing passages) can be changed to correct imperfections either present from birth or resulting from trauma. Blepharoplasty (Eyelids plastic surgery)

Will having blepharoplasty affect how my contact lenses fit?

Blepharoplasty surgery only works on the skin and muscle of the eyelids
Area. Although you won´t be able to wear contact for the first few
days after surgery (to allow time for the healing process to begin) blepharoplasty surgery will not change how your contact lenses fit or function.

How many types of facelifts exist?

There are several names and variations associated with the facelift procedure. Patients with varying facial conditions will need different
types of facelift.

The different procedures are meant to address the three different thirds of the face. A brow lift, forehead lift and blepharoplasty target the upper third of the face, while a midface lift will address the cheek area and lower portion around the eyes. A full facelift will address the mid and lower cheek area, jawline and neck zone.

Facelifts are usually performed along with blepharoplasty as the eyelids are one of the components of the face that first present aging signs.

A facelift alone however, may not be enough to make certain wrinkles and folds disappear completely. Micro fat grafting or any other dermal filler may be used together with the facelift to address deep wrinkles such as the nasolabial fold.

I would like to improve the look of my neck, not my face. Do I need a facelift?

While a “necklift” alone may correct and restore your neck area, it is very likely that you will later start noticing the aging signs on the rest of your face as we tend to age from forehead to neck; from the upper third of the face to the lower, and not the opposite.

Is 35 or 40 too young for a facelift?

Good candidates for a facelift can indeed be as young as 35, as there are various types of facelifts that address different areas of the face. Aging signs generally appear first in the upper third of the face, such as drooping eye brows, puffy eyelids or a cheek starting to droop. A midface lift or forehead lift can be a merely preventive or maintenance surgery.

What´s the difference between a traditional and an endoscopic forehead lift and brow lift?

The main difference lies in the type of incision made and the endoscope used as a viewing tool. In a traditional forehead lift the surgeon makes a coronal (headphone-shaped) incision behind the hairline, stretching between the ears and across the top of the forehead. In a minimally invasive endoscopic foreheadlift the surgeon makes three to five short incisions (less than an inch long) behind the hairline. The endoscope, a slim instrument with a camera on the end, is placed in one incision so the surgeon can see beneath the skin, while he lifts the skin and adjusts muscles through the other incisions.
A browlift can be performed with an endoscopic approach as well.

Is Botox or a facelift better?

Being not such a thing as “better”, it is important to mention that besides being Botox applications and a facelift completely different procedures one from the other, Botox is a short term solution as opposed to a facelift which will yield a long lasting result.

Botox is injected into your face with a needle to paralyze specific muscles on your face, to reduce the appearance of wrinkles. A facelift makes minimal incisions in your skin to gently pull and lift the wrinkles out. Facelifts require anesthesia and minimal incisions, but the effects last longer. Botox is a simple procedure without anesthetic, but require visits more often to keep up the results and you may end up paying the same or more for Botox applications in the long run than for a facelift.

The same applies for dermal fillers as a substitute for a necessary or preventive facelift.

Dermal Fillers

There are different types of dermal fillers intended to treat the different surfaces of the skin. Some common brands are Juvederm, Aquamid, and Restylane.

Yes. RESTYLANE™ has been used safely by dermatologists and plastic surgeons since 1996 worldwide. Because RESTYLANE™ is non-animal-based and fully biocompatible, allergy testing is not required. The most commonly reported side effects were temporary redness and swelling at the injection site. These effects typically resolve within two to three days.

How is RESTYLANE™ different from BOTOX® and collagen?

RESTYLANE™ is a non-animal-based natural cosmetic filler. BOTOX® is a drug that blocks nerve transmission and relaxes the underlying muscles.

Bovine collagen is derived from animals and requires an allergy test before it can be used. Because RESTYLANE™ is a non-animal-based hyaluronic acid, it can be administered without pre-testing. In addition, it delivers a longer-lasting effect than bovine collagen. Dr. Bukret does not use bovine collagen.

Breast Augmentation

How much can my breasts be enlarged? What is the limit of this? Why is it limited?

The degree of enlargement is based on the anatomy of the patient´s body more than any other factor.

The width of the patient´s chest, as well as the locations of nerves in the chest and abdomen limit the maximum volume of an implant that can be used, because the total width of the implant must fit underneath the muscle.

For some patients, placement of the implant may be possible on top of the muscle layer, provided that they have a sufficient amount of glandular and fatty tissue to give the implant the coverage needed to achieve natural results.

After breast augmentation:

Will I be able to breast feed?

Breast augmentation should not affect your ability to breast feed.

Will my breasts still be saggy?

Sagging of the breast is a factor determined by your body. Some degree of sagging can be corrected with the breast augmentation surgery, but not an excessive degree. For an excessive degree of sagging, a mastopexy or ´breast lift´ may be the best choice.

Will my breasts sag later?

In general, if the factors which caused the sagging in the first place (heavy breast tissue, lack of elasticity of the skin, etc), are still present, then the breasts will again begin to sag over time. If correcting this sagging is of primary importance to you, you can discuss with Dr. Bukret other alternative surgeries (such as mastopexy) which may be done instead of, or in addition to the breast augmentation procedure.

Laser Liposuction & Liposculpture

How long will it take for the swelling to subside?

You will be able to appreciate the final result of your body liposculpture or liposuction within a period of three months. Each month after surgery the swelling subsides about a 30%. Patience is a virtue you will find very useful after having liposuction.

Will the plastic surgeon leave the incisions open? Why?

Yes, in most cases incisions are left open to aid swelling subside, as it will take strain off your body metabolizing the tumescent solution. The first few hours after surgery you will experience heavy draining and staining of your sheets and compression garment. The good news is this will only last some hours.

Can a secondary liposuction be performed?

Yes, although it is recommended to be performed three months after your prior liposuction.

Can Abdominoplasty eliminate “stretch marks” from pregnancy?

If the stretch marks are located in the incision area for the surgery (below the navel) then they can be removed in the course of abdominoplasty. In locations not at the incision, site, they cannot be removed, although it is sometimes possible to use a different incision location to accomplish this goal.

Surgery on just the stretch marks alone will merely trade the stretch marks for an incision scar, and is a generally a poor trade-off considering the cost and effort of surgery.
